Health Check and Insurance for Migrant Workers

A work permit renewal will not go through without the health check and health insurance in place. Exworker takes care of the health side end to end — from checking which document your employee can present at the hospital, to choosing a facility whose results feed into the Department of Employment's system, to arranging health insurance that covers the round you are filing for.

None of this is difficult, but it is the bottleneck of the renewal round, because everyone has to pass through it and hospital queues lengthen across the whole country at the same time as the deadline approaches. Employers who start late are the ones who find the queue full.

Information as of August 2026

The point that saves the most time: you do not have to wait for the new card

The Department of Employment confirmed this on 13 August 2026 Foreign nationals renewing their work permit through to 11 December 2027 may present the work permit, or any document or evidence issued by the Department of Employment showing permission to work until 11 December 2026, and go for the health check straight away — there is no need to wait for the new card. The Department has notified the Ministry of Public Health and the Bangkok Metropolitan Administration Medical Service Department accordingly.

This matters because it is where clients lose the most time — taking an employee to the hospital only to be turned away because "the card is not out yet", when under this letter there is no need to wait. If that happens, cite the Department of Employment letter dated 13 August 2026, and check whether that facility appears on the list the Department publishes.

Choose the facility on the right basis, not the nearest one

What to look for Results linked to the Department of Employment's system

The Department publishes a list of private facilities that meet its criteria and link health check results to its system — 69 in total as at announcement no. 16, dated 19 August 2026.

Read it correctly It does not mean only those 69 will do

That list covers private facilities meeting the criteria; it is not an exclusive list — public facilities are a separate channel. Before taking anyone in, check with the provincial employment office and with the hospital itself, every time.

What we do not state The examination fee and the tests included

The Department's announcement sets neither the fee nor the tests, so we do not print figures that could mislead. We tell you the rate at the facility chosen, case by case.

Health insurance — choose the term that covers the filing date

The price box above is split into two terms, because clients choose according to the time actually left for the employee rather than buying both.

Health insurance, 1 yearFor employees staying on across the renewal round. Better value than short terms and no need to come back mid-round.
Health insurance, 6 monthsFor cases with a short remaining period, or where the next round is not yet clear — we work out which term is better value for each employee.
Health insurance and social security are not the same thing An employee already in the social security system and an employee who must buy health insurance under the Department's conditions are in different positions. Send us the employee details first and we will tell you what your case needs, so nothing is paid for twice without reason.

What happens when we handle it

  1. Document check first (free) Which document each employee holds, and whether it can be presented for the health check as it is or has to be fixed first.
  2. Choosing the facility and booking One whose results link to the Department's system and that is realistically reachable, with its charges told to you beforehand.
  3. Arranging insurance to cover the filing round One year or six months, according to the time actually left.
  4. Handing over into the renewal The results and the policy go straight into the document set used to file the work permit renewal.
This much is enough to start with A photo of the work permit (or the document the Department issued) and the employee's passport page, plus the province and how many people. We will reply with which document can be presented, where to go, and which insurance term to take.

Why employers have us cover the health side too

  • We check in advance that the document the employee holds can actually be presented, so nobody gambles at the hospital counter
  • We choose a facility whose results can enter the Department's system, rather than a check that cannot be used to file
  • We schedule the health work ahead of the filing queue, so the two do not collide near the deadline
  • We work out which insurance term is better value per employee, instead of selling everyone the long one
  • Several employees can be handled at once, with transport and appointments arranged as a batch

Frequently asked questions

The new card has not arrived — can the health check still be done?

Yes. The Department of Employment's letter dated 13 August 2026 allows the work permit, or a document or evidence issued by the Department showing permission to work until 11 December 2026, to be presented for the health check. If a hospital refuses because the card is not out, cite that letter.

Can the check be done anywhere?

It should be done at a facility whose results can link into the Department of Employment's system. The Department publishes a list of private facilities meeting its criteria — 69 as at announcement no. 16 (19 August 2026) — but that does not mean only those will do. Check with the provincial employment office and the hospital before going.

How much is the health check?

The Department's announcement does not set a fee, so we do not quote a general figure that could mislead. We tell you the rate at the chosen facility before taking anyone in.

What is tested?

The list of tests is not prescribed in the announcement of approved facilities, so we confirm it with the booked facility case by case rather than listing tests loosely.

We already have social security — is health insurance still needed?

They are different positions and it has to be assessed per person. Send us the details and we will tell you what your case needs, so nothing is paid for twice.

When should it be done to be in time?

As early as possible, because everyone in the same group has to be checked before filing, so hospital queues lengthen together as the deadline nears. We always schedule the health work ahead of the filing queue.
